【 要旨 】 内視鏡的乳頭括約筋切開術（endoscopic sphincterotomy；EST）は総胆管結石に対する標準的治療として行われるほか，各種胆膵内視鏡治療や診断に際して適応がある．ESTを安全かつ確実に実施するためには，基本的な指針が必要である．日本消化器内視鏡学会は，科学的な手法で作成した基本的な指針として，「EST診療ガイドライン」を作成した．本ガイドラインでは，適応，手技，特殊な症例への対処，偶発症，治療成績，術後経過観察の六つの項目について計23個のClinical Question（CQ）を作成し，各CQに対するステートメントと解説文という形で構成されている．本稿では本ガイドラインに基づいた診療の実際について述べる．

[ Summary ] Endoscopic sphincterotomy (EST) is indicated as the standard treatment for common bile duct stones, as well as for various endoscopic diagnoses and therapy in pancreatobiliary diseases. It is important to establish clinical guidelines to perform EST safely and accurately. Japan Gastroenterological Endoscopy Society has recently compiled the "Clinical Guideline for Endoscopic sphincterotomy" as fundamental guidelines using evidence‒based methods. The working committee established 23 clinical questions (CQ) from 6 sections including indications, techniques, special cases, complications, outcomes, and postoperative observation. The guidelines are consisted with statements and comments regarding each CQ.
