【 要旨 】 大腸カプセル内視鏡の下剤には，次の二つが必要となってくる．一つ目は大腸カプセル内視鏡服用前にその画像視認性を高めるために行う腸管洗浄としての下剤，二つ目は大腸カプセル内視鏡服用後の後押し（ブースター）として用いる下剤である．便汁で混濁した腸液で満たされる腸管内では，たとえそこに病変があったとしても画像でそれをとらえることはできない．また，途中でバッテリーが終了した場合には，そこから肛門側は観察できない．大腸カプセル内視鏡服用前の下剤で良好な腸管洗浄度を確保し，かつ大腸カプセル内視鏡服用後の下剤でバッテリー時間内にカプセルを体外に排出させることが重要となってくる．本稿ではこの下剤服用の工夫について概説する．

[ Summary ] PillCam colon capsule endoscopy (CCE) is a very recent diagnostic technology designed to explore the colorectum. CCE‒2, referring to second‒generation CCE, has a sensitivity of 84‒94 % for detecting polyps with a diameter of > 6 mm. Since colon cleanliness is essential for optimal visualization during colon capsule endoscopy, adequate bowel preparation may require as much as 6 L of fluid intake over two days, which may be considered excessive for some patients. Reducing the volume of fluid for bowel preparation is an important requirement in increasing patient acceptance of CCE. In this section, we explain how a laxative can be effectively used in bowel preparation to achieve high quality visualization and how it can be used as a booster to ensure that the capsule is excreted within its battery life period.
