【 要旨 】 胆道閉塞と十二指腸閉塞をともにきたす症例における姑息的治療は，おのおの単独で行う場合と異なる制約が存在することがある．比較的古くから行われてきた外科的バイパス術や経乳頭的胆道ドレナージに加え，through-the-scopeでの内視鏡的消化管金属ステント留置術，超音波内視鏡ガイド下胆道ドレナージによる非経乳頭的一期的内瘻術，細径化された経皮経肝胆道金属ステントによる一期的内瘻術などが実現可能となった．患者の病態に応じてこれらの多彩な治療法を組み合わせ，戦略的に治療順序を組み立てる必要がある．

[ Summary ] Palliative double stenting for biliary obstruction and duodenal obstruction is complicated and challenging compared to single stenting for either of these two obstructions. Duodenal stenting with a self-expanding metal stent which can be released through the working channel of an endoscope, endosonography-guided hepaticogastrostomy and choledochoduodenostomy with a metal stent, and percutaneous transhepatic biliary stenting with a self-expanding metal stent in a single session have recently become available. Various methods for double stenting utilizing these newly developed techniques are illustrated.
