【 要旨 】 超音波内視鏡下胆管ドレナージ(EUS-BD)は,ERCPが不成功に終わった際の代替胆管ドレナージ方法として,近年多くの報告を認めている.EUS-BDは大きく三つの手技に大別される.(1) 消化管と胆管の間にEUS下に瘻孔を形成するEUS下胆管消化管瘻孔形成術,(2) EUS下に胆管を穿刺したのちに,ガイドワイヤーを胆管・乳頭を介して十二指腸に留置し胆管カニュレーションを行うEUS下ランデブー法,(3) EUS下に消化管と胆管の間に一過性に瘻孔を形成し,胆管閉塞部にステントを留置するEUS下順行性治療がある.いずれの方法においても,EUS下処置とERCPの要素を含んでおり,どちらの手技にも精通した術者により行われるべきであり,EUS-BDが不成功に終わった際のPTBDや外科的処置が施行できる後方支援も合併症を最小限にするために重要である.

[ Summary ] Endoscopic ultrasound-guided biliary drainage (EUS-BD) has recently emerged as alternative drainage technique after failed ERCP. EUS-BD is divided into 3 different techniques. 1) EUS-guided transluminal biliary drainage : a fistula is created for biliary drainage between the intestine and the biliary duct under EUS guidance. 2) EUS-rendezvous : a guidewire is placed through a FNA needle, the biliary duct and ampulla and into the intestine under EUS-guidance, followed by biliary cannulation. 3) EUS-guided antegrade treatment:stent placement is performed through a temporal fistula which is created between the intestine and the biliary duct under EUS-guidance. All 3 EUS-BD techniques require endoscopistʼs expertise in both EUS and ERCP procedures, since these procedures contain technical factors used in EUS and ERCP. The availability of alternative biliary decompression, such as PTBD or surgery, is also crucial to minimize the risk of complications in preparation for unsuccessful EUS-BD.
