【 要旨 】 | 乳癌肝転移は一般的には,”全身病”との認識のもと,ホルモン療法,化学療法が選択される.子標的薬も登場したが,平均生存期間は1〜2年,5年生存率は10%以下と,その予後は依然不良である. そこで,われわれは,肝切除とマイクロ波凝固壊死療法(MCN)を用いることで,まず”volume reduction surgery”を行い,可能なかぎり肝転移を制御した後に化学療法を行うことで,予後の延長が可能ではないかと考えた.27例にこの方針にて治療にあたり,結果,肝転移初回手術からの5年生存率は51.0%,平均観察期間54.9カ月と非常に良好な結果を得た. 乳癌肝転移に対する外科治療は一般的ではないが,治療成績は良好で,他施設の報告も併せ,紹介したい. |

[ Summary ] | From July, 1994 to November, 2009, we surgically treated 27 cases of liver metastases resulting from breast cancer. Resection and/or microwave coagulo-necrotic therapy (MCN) were performed. After these operations, chemotherapy and/or endocrine therapy were also given to the patients. The 1, 3, and 5-year survival rates for all patients after their first surgical liver treatment were 96.2 %, 60.2 %, and 51.0 %. In general, breast cancer metastases into the liver are rarely indicated for surgical treatment. However, we have recently encountered reports that liver resection is a useful procedure to improve the poor prognoses for liver metastases of mammary origin. From related literature and our experience, we believe that surgical treatments such as reduction surgery should be considered to be beneficial options for liver metastases arising from breast cancer. In particular, MCN is a useful procedure for multiple liver metastases. |
