【 要旨 】 | H.pyloriの感染胃粘膜において,内視鏡的に認められる胃底腺粘膜のびまん性発赤を,定量的に捉えるHemoglobin index(IHb)値による除菌判定を試みた. H.pylori感染のIHbのcut off値を60に設定すると,除菌成功例では,除菌1カ月後より有意に低下を認め,3カ月後には全例60未満の値を示し,再上昇例は認められなかった.除菌不成功例では,除菌1カ月後にIHbの低下した例も認められたが,3カ月後には再び高値を示し,それ以降もほぼ除菌前の高値が持続した. IHbによる除菌診断はcut off値を60に設定すると,3カ月後には感度,特異度ともに100%となり,除菌診断に有用と考えられた. |

[ Summary ] | Diffuse redness of the fundic mucosa of the stomach was characteristic of H.pylori (Hp) infection. The hemoglobin index (IHb) can quantitatively express the grade of the diffuse redness of the gastric mucosa. Endoscopic diagnosis of Hp eradication was tried for IHb measurement. The IHb value for cases with successful eradication fell below 60 three months after eradication and lowered values continued thereafter. In some cases of unsuccessful eradication, IHb values fell below 60 one month after HP eradication. However, IHb values continued to be greater than 60 three months after eradication. Determining the cut off index for IHb to be 60, the results of diagnoses were determined to have a sensitivity and a specificity of 100% respectively. |
