Theme Strategy of managing small colorectal polyps
Title Morphologic changes of colorectal adenoma by follow-up of Barium enema findings

[ Summary ] 272 colorectal adenomas in 182 patients were investigated by means of follow-up examinations with Barium enema, over an interval of a year or more. All adenomas in this study were histologically proven by the specimen of biopsy or surgical procedures. The age distribution ranged from 27 to 79 years old and male to female ratio was 114 male(184 adenomas) and 68 female(88 adenomas). These adenomas were classified into three groups depending on their size, which was measured during the initial Barium enema. The smaller group consisted of adenomas 5mm or less in size. The middle group was between 6-10mm and the large group was over 11mm in diameter.
The average observation period in this study was 4.55 years.
We calculated that 143 adenomas(52.6%) showed no change in size between the initial and the final Barium enema. 97 adenomas(35.7%) showed increase in size and 32 adenomas(11.8%) showed reduction in size. However, as far as the change rate is concerned, the small adenomas represented 63.8% , the middle size group represented 34.1% and the large group represented 45.1%.
The results of this study we may conclude the following:
1. Increase in size had no prediction for a specific site in the colorectal tract.
2. More than 90% of adenomas showed no change in size or morphological configurations.
3. The doubling time for adenomas is estimated at 60.1 months.
4. In adenomas over 6mm in size, the doubling times of those which were semi-spherical or spherical in shape were 53.6% and 84.3% respectively.
