【 要旨 】 |
近年の超音波(ultrasonography;US)装置の空間分解能の向上により,体外から腸管を評価することが可能となった.体外式腹部US 検査は,安価,低侵襲で前処置不要,反復施行が可能で,欧州を中心に炎症性腸疾患(inflammatory bowel disease;IBD)の再燃の有無,疾患活動性のモニタリングに広く用いられている.体外式腹部US検査による潰瘍性大腸炎(ulcerative colitis;UC)のモニタリングでは,おもにUSで測定した腸管壁厚によりUC再燃の有無を,またUSによる層構造および壁内血流評価により重症度のモニタリングを行っている.既報において,UCの内視鏡的再燃の有無および内視鏡的重症度と,US 所見の相関は高いと報告され,UC活動性モニタリングの低侵襲で有用なツールとしての役目が期待される. |

[ Summary ] |
Advances in the spatial resolution of ultrasonic imaging devices have enabled evaluation of the gastrointestinal tract from outside the body. Transabdominal ultrasonography (US) has the advantages of being a low-cost, non-invasive, easy-to-perform procedure that does not require any prior preparation. US is useful for monitoring flare-ups and disease activity in inflammatory bowel disease. In ulcerative colitis (US), US can be used to monitor disease flare-up by measuring bowel wall thickness, and to assess disease severity by evaluating bowel wall stratification and blood flow pattern. A high correlation has been found between US and colonoscopy findings. Thus, US helps to monitor disease activity in a non-invasive manner. |
