[ Summary ] In most countries, the histopathological diagnosis of colorectal carcinoma has been performed using figures derived from submucosal invasion of tumorous glands. Regarding this standard, mucosal carcinomas can not logically exist. But biologically colorectal carcinomas have arisen from the mucosa, and to investigate the developmetal process of colorectal carcinomas, mucosal carcinomas should be diagnosed histopathologically.
The histological standards for mucosal carcinoma were different from those of gastrointestinal pathologists in Japan. For purpose of standarizing the criteria, the measurment of atypical degrees of histological figures was examined using several compute analyzers. Some indices of atypical degree, in which the rate of glandular area occupied a view (index of structural atypia ; ISA), the rate of nuclear area occupied in one gland (index of nucleusgland ratio ; ING) and the rate of nuclear inarrangernent (index of nuclear arrangement ; INA), had been obtained. From the data examined, the criteria for atypical degree of mucosal carcinoma was that, ISA was more than 80% and ING was more than 50%. We examined the small superficial type of submucosal invasive carcinomas, the data showed an incidence of nuclear atypia of IIc type which was higher than those of other types, this was mostly related to their invasive depth. So in spite of macroscopical figures, mucosal carcinomas can be diagnosed by using the criteria above-mentioned. Also the discriminant Fca=0.08(ING)+0.04(ISA)-6.59 was used for differentiating carcinoma from adenoma.
The indices of atypical degree were useful when diagnosing mucosal carcinoma, and should be investigated to compare with immunohistochemical examinations.
